Quality of Life With Urinary Diversion

چکیده
There continues to be debate about how best to replace bladder function following radical cystectomy. All the usual options currently involve the use of transposed intestinal segments, and all have advantages and disadvantages. The impact of each option on quality of life (QoL) is an important consideration. This review assesses the evidence base that underpins each of themajor forms of transposed intestinal segment surgery (ileal conduit diversion, continent urinary diversion, and orthotopic bladder replacement). The meaning, determinants, and measurement of QoL are also explored. We conducted a systematic review searching Medline, PubMed, EMBASE, CINAHL, and the Cochrane Library from 1966 to August 2010. A total of 46 studies (including 8 prospective studies) met the inclusion criteria reporting on 4186 patients. Although the reports suggest a reasonably well-maintained QoL after all transposed intestinal segment surgical options, most were retrospective studies, and the few prospective nonrandomised studies had limitations. From the available published evidence, it still remains unclear if one form of transposed intestinal segment surgery is superior to another in terms of QoL. In light of this continued uncertainty, there is an urgent need to establish the important determinants of QoL for this cohort; to standardise a set of core important outcomes; and, once and for all, to undertake high-quality prospective (ideally randomised) studies comparing the different surgical options to help guide policy and clinical practice in transposed intestinal segment surgical practice. Until such time, care will continue to be individualised, taking into account patient factors, surgeon factors, centre factors, and health provider factors, unsatisfactory as this may be for patients. # 2010 European Association of Urology. Although it has been reported that continent urinary diversions may not have increased morbidity in the short term,9 practically speaking, these diversions do require more effort and diligence than an ileal conduit. Ultimately, it is a very personal patient decision as to the type of ur...
متن کاملImpact of invasive bladder cancer and orthotopic urinary diversion on general health-related quality of life: An SF-36 survey
Bladder cancer is a common type of genitourinary cancer, and radical cystectomy with urinary diversion is considered to be the most effective local treatment for invasive bladder cancer. In order to assess the functional results and health-related quality of life (QOL) in bladder cancer patients with an orthotopic neobladder, and to provide a reasonable basis for the evaluation of urinary diver...

AIM To investigate quality of life (QoL) domains with three forms of urinary diversions, including ileal conduit, MAINZ pouch, and orthotopic ileal neobladder after radical cystectomy in men with muscle-invasive bladder cancer. MATERIALS AND METHODS In a prospective study, 149 men underwent radical cystectomy and urinary diversion (70 ileal conduit, 16 MAINZ pouch, and 63 orthotopic ileal neo...
متن کاملRe: Orthotopic Neobladder vs. Ileal Conduit Urinary Diversion: A Long-Term Quality-of-Life Comparison.
PURPOSE The optimal form of urinary diversion following radical cystectomy remains controversial. We sought to compare the long-term health-related quality-of-life outcomes between patients with orthotopic neobladder and ileal conduit diversion (ICD). PATIENTS AND METHODS We enrolled 95 patients following radical cystectomy and ICD (n = 49) or orthotopic neobladder reconstruction (ONR) (n = 4...

There continues to be keen interest in assessing quality of life (QoL) after cystectomy and urinary diversion, given the recent evidence-based analyses [1,2] and systematic reviews [3–6] evaluating QoL studies in this population. A resounding characteristic of contemporary reports is the preponderance of cross-sectional, retrospective, single-institution studies. Very few prospective longitudin...
متن کاملHealth-Related Quality of Life after Cystectomy and Urinary Diversion for Bladder Cancer
With multiple options for urinary diversion after radical cystectomy for bladder cancer that have comparable cancer control and complication rates, health-related quality of life (HRQOL) has become an important consideration. This article reviews the methods for defining HRQOL, the challenges in measuring HRQOL in bladder cancer, and the literature comparing HRQOL after various methods of urina...
